LVPC Member's Liability Waiver and Event Agreement

The Lehigh Valley Photography Club (hereafter “LVPC” or “Club”) continues to monitor policy and procedures related to the health and welfare of our membership, including but not limited to COVID-19 or potential physical injury during club activities. Please review the Member Agreement below. Your acceptance of these terms will be required to register for LVPC in-person events.

As the safety and well-being of our members and participants (including guests and members’ families) is our top priority, we follow the guidelines of federal, state, and local agencies, as well as utilized facilities to implement best practices aimed at managing the risk of injury or the spread of viruses and any other communicable diseases.

With regard to communicable diseases, LVPC uses preventative measures when possible to help reduce potential exposure; however, the Club, under no circumstances, can guarantee that you or a guest will not become infected with any virus or disease while at Club activities/events and in surrounding areas.

Member Agreement:

• I declare I will not attend/participate if I am experiencing or exhibiting any symptoms of a known or suspected communicable disease or virus, including COVID-19, as outlined by the Center for Disease Control and Prevention (CDC). Examples: fever, dry cough, or shortness of breath.
• I acknowledge I must follow the safety protocols that are currently implemented by LVPC and the locations and facilities that it utilizes for activities and events which may include protective clothing or footwear, practicing social distancing, use of masks, and/or any vaccination as legislated by federal and/or state government(s).
• I understand and accept that the guidelines provided by the Club, authorities and facilities may be modified and changed with time in accordance with evolving guidelines and/or legislation and I will follow the most current guidance.
• I will not attend if I have traveled to any area that has been heavily impacted by any communicable disease, internationally or within the United States, within 14 days prior to the event.
• I will not attend if I am aware or believe that I have been directly exposed to any communicable disease within 14 days prior to the event.
• I will not attend if I have tested positive for COVID-19 or any other disease that is communicable by casual contact within the last 10 days.
• I acknowledge I may be removed from an event or asked to leave an event by LVPC, or facility personnel should I exhibit any behavior in direct contrast to LVPC or facility guidelines and that I will abide by those requests and directives.
• I agree that a Club member and/or the guest(s) of a member attending the event cannot be a minor child under the age of eighteen years.


I also recognize that various events and/or activities may involve risk of injury. I recognize that participation in any event may include (but not limited to):

• travel to and from event locations and sites;
• physically demanding activity over potentially wet, slippery, or rough terrain;
• the possible reckless conduct of other participants, LVPC members and nonmembers, in the event   and acknowledge that unforeseen risks may be encountered.

I voluntarily agree to assume all the foregoing risks and accept sole responsibility for any expense, liability, illness, injury, disability, or death related to contracting the virus or other injuries while attending an event organized by LVPC. I hereby release, covenant not to sue, discharge, and hold harmless LVPC from all claims of any kind arising out of exposure to any communicable diseases, accidents, or other injuries. I understand and agree that release of liability includes any claims towards LVPC, Club personnel, representatives, members, and officers whether any health infection occurrence exists before, during, or after attendance and participation at an LVPC event, or if I am otherwise injured.

This event attendee waiver and release of liability form shall be governed by and construed under the laws of the State of Pennsylvania, without regard to its choice of law principals. I agree not to commence or prosecute any action in connection herewith other than in the state and/or federal courts of the State of Pennsylvania.
